Item entities are drawn as full models, which costs more than the little sprites spinning on the floor suggest. Fast Items cuts that cost by transforming items into 2D forms, borrowing the flatter look that popular clients use for items on the ground.
Items that are already flat, such as potions, are drawn without their sides, leaving them completely flat instead of an extruded slab. Genuinely three-dimensional items like cube blocks are handled differently: rather than showing their geometry from every angle, they are turned to always face the camera, the same behaviour found in Lunar Client and BLC.
Four settings control the whole thing. The mod itself is on by default, as are entity shadows for items and the handling of 3D models such as blocks, while side rendering for 2D items starts off. Flip any of them to strike your own balance between the vanilla look and the faster flat rendering.
