Pommel hands resource pack creators the trick Minecraft already uses for the spyglass and the trident: an item can keep its familiar look in the inventory and appear completely different the moment it is in your hand. A mace can gain a fully three-dimensional model, a sword can turn into a fiery greatsword, and a plain tool can become an ancient magical staff, while the original item itself is left untouched.
The mod works entirely through item predicates that a model file can react to. Packs can target an item being held, shown in first person versus third person, sitting in the offhand slot, placed in an item frame, worn in the helmet slot, dropped on the ground, thrown, being used, submerged in water, enchanted, or held by villagers, witches, pandas and foxes. Each of these can point to its own model file, and the held models and their textures may live in any folder as long as the item model file references them.
Some predicates go further than a single swap. The use-duration predicate follows how far along an action is, so an apple can step through several models while it is being eaten, including a set of dedicated in-hand versions. The is-used state syncs across multiplayer when the server and the players both run the mod, and both the use and submerged states also change the icon shown in the inventory. Modded items are supported the same way as vanilla ones, so a pack built for a large mod list can restyle those items too.
