Held items normally stay tucked away out of sight, which makes it hard to tell at a glance what your team is actually carrying. This add-on draws them right on the Pokémon in the world, so a held berry or tool is visible on the creature itself. Cobblemon has to be installed for any of it to work, since this is purely a companion mod.
Multiplayer is covered as well. On a server that is not running the mod, only your own Pokémon’s items get drawn, so it stays usable anywhere. When the server does run it, administrators keep control through a data pack: replacing the item tag lists changes which items are hidden and which ones count as hats or eyewear. Hidden items are still shown to the Pokémon’s owner, and any vanilla or modded item can be added to those lists, so packs can tune the look exactly the way they want.
Model and Fakemon authors get their own hooks. Adding a held_item locator makes the item sit the way a player holds it, which suits spyglasses, tridents and similar tools, while held_item_fixed falls back to the flat item model that works better on bird- or dog-like creatures. Rotating the locator changes the angle of the item, held_item_head and held_item_eyes place wearables on the head or over the eyes, and a null object carrying a scale value resizes anything that comes out too big or too small.
