Carrying a Totem of Undying in vanilla Minecraft always costs something: either a hand you would rather use for a tool, or an inventory slot you have to keep clear for it. Charm of Undying removes that trade-off by adding a new Charm slot to the player inventory, meant for exactly this item.
Once the totem sits in that slot, it protects you from death the same way it would while held, so the effect works without the totem taking up space in the hotbar. Getting it there is simple: move it into the Charm slot from the inventory, or use the totem while holding it and it moves in on its own. After that there is nothing to remember and nothing to swap in the middle of a fight.
The slot comes from the accessory frameworks the mod is built on, the Curios API on Forge and the Trinkets API on Fabric and Quilt. The project was released earlier under the names Curio of Undying and Trinket of Undying, so those older names point to the same mod.
